People,

I ran across the following sentence:

"Hizo trisas el florero".

I think that something is strange here. It stipulates that a person, third person singular, broke the flower vase.

I think that more common would be: "Se hizo trisas el florero". Right?

If a person did it, third person signular, wouldn't the preposition "a" be necessary? "Hizo trisas al florero".

If I had broken it, I think that I would say: "¡Ay! Se hizo trisas. Perdone".

"Se hizo trizas el florero" is an example of passive "se": it implies that the damage happened by itself or accidentally.

No, "a" would not be used here. Using the preposition "a" before a direct object typically happens when the noun is a definite noun AND the noun refers to or names a person or personified being. Otherwise most of the time no preposition is used.
