I understand this expression as when you refer to some piece of art, (a poem, a speech, a painting... a video) that really affects you profoundly, and really touches your heart...

Is that a correct interpretation?

Or is it something like it hits you on your "forefront" and/or your "brain" and you get it, having a total realization, and/or understanding of the matter at hand?

It reminds me the pun on the baseball... (or soccer ball...)

The grand opening of our store hit the competition right between the eyes. (complete surprise)
... and then it hit me (between the eyes) = ... and then it became apparent (and then I realized)

The figurative sense of complete surprise has a synonym: to knock the socks off of someone (to knock someone's socks off)
