Translation Request... Help! :)
 
Hello, my name is Sarah and I am trying to learn more Spanish to help me in my daily job duties. I am pretty decent at asking questions, but I have trouble receiving answers and I also think that the questions I'm asking are a little too formal. So, I was hoping that someone could help me out with some less formal tips? I appreciate any help you can give.

Below I have written out an average conversation that I have with a patient at work. If possible, could someone help me translate these questions into a less formal format? I have also written in red, how I normally say things.

Hola. Qual es su direction?

So... as you can see, my knowledge is pretty basic and I could really use some help. I'm leary of using things like Google Translate because they give you horrible answers. Help! Thank you in advance!

Blue = correction / translation
Green = simplification / less formal

Hola. ¿Cuál es su dirección?

What is your social security number?
Qual es su numero du social? :bad:
¿Cuál es su número de seguro social? --> ¿Su número de seguro social? (you don't need to say "Cuál" again if they're one question after another)

What is your date of birth?
Qual es su fecha de nacimiento? :good:
¿Cuál es su fecha de nacimiento? --> ¿Fecha de nacimiento?

Are you single or married?
Esposa/esposo? :good: (This is enough if you're filling an inquiry?/a query? (I'm pretty lost with the meaning of these terms o.o)
¿Es soltero o casado? (M) / ¿Es soltera o casada? (F)

Who is your regular doctor?
Quien es su doctor? :good:
¿Quién es su doctor?

Are you currently working? Where do you work?
Trabajas? Donde? :good:
¿Trabaja? ¿Dónde? ("Trabajas" shouldn't be used unless your patient is younger OR if you know that person OR if the place environment is familiar")

¿A quiénes podemos registrar como sus contactos de emergencia?

¿Cómo están relacionados (el/los contacto/s de emergencia con usted)?

Are we billing insurance for you today? (I don't really get the meaning of this question... are you asking if the patient have insurance OR you're asking him if he is going to charge it to his insurance?)

Accidente ou trauma? :bad: (This is like asking the patient if he WANTS one of them)
¿Está aquí debido a un accidente o lesión?

Where did it happen?
Donde? (Dónde = Where... so where what[happen]?)
¿Dónde sucedió?

When did it happen?
Quando? (Now it's okay to only ask "when" because you already asked the 'what[happen]' before)
¿Cuándo?

Do we have your permission for the doctor to treat you today?
¿Tenemos su permiso para que el doctor lo trate hoy día?

And permission to bill your insurance?
¿Y permiso para cobrar su seguro?

And you understand there will be a bill for this visit?
¿Y entiende que se le cobrará esta visita?

This form states that we will bill your insurance, but do not know what they will or will not pay.
Esta forma declara que le cobraremos su seguro, pero que no sabemos lo que ellos están dispuestos a pagar o no.
