No comma needed, otherwise you are correct. The number comes before the the street name except in rare cases.

Poli is right regarding the US Postal Service recommended format. If the address includes an apartment number, unit number or suite number within the building it appears on the same line right after the building's address.

(All caps is a USPS standard. No comma, no period.)

If there's a unit number, an apartment number or a suite number, it appears on the same line as the street. No punctuation is used.

There are many abbreviations that can be used in a street suffix:

AVENUE (AVE)
BOULEVARD (BLVD)
CENTER (CTR)
CIRCLE (CIR)
DRIVE (DR)
HIGHWAY (HWY)
LANE (LN)
MOUNT (MT)
PARKWAY (PKWY)
PLAZA (PLZ)
POINT (PT)
ROAD (RD)
ROUTE (RTE)

I live at 75 Fifth Avenue.
The school is located at 10 Church Street, next to the church.

I live on Fifth Avenue. What street do you live on?

What's your address?
988 Valley Lane.
