How would you render something like this in Spanish?

"I hereby authorize and consent to the above named procedure to be perfomed by Dr. X."

My suggestions:

Por la presente autorizo y doy consentimiento al Doctor X que lleva a cabo el procedimiento mencionado anteriormente.

-or-

Por la presente autorizo y doy permiso al Doctor X que lleva a cabo el procedimiento sobredicho/antedicho/supradicho.

Rusty's are OK, but it's more common to change the order.
Por la presente consiento y autorizo al Dr X a realizar el procedimiento arriba indicado.
Por la presente consiento y autorizo al Dr X a llevar a cabo el procedimiento arriba
indicado.

I hope Dr X has not a sexual meaning

... autorizo a que lleve a cabo... Autorizar a alguien a/para hacer algo. Si en lugar del infinitivo aparece un verbo personal éste ha de ir en subjuntivo.

Antedicho se utiliza, pero sobredicho y supradicho no son nada comunes.

Las traducciones de Rusty son buenas si se tiene en cuenta lo anterior. Por ejemplo:

Por la presente autorizo y doy mi permiso al doctor X para que lleve a cabo el procedimiento antes mencionado.

Autorizar y dar permiso es redundante, pero es una fórmula utilizada en este tipo de registro.
