Hasta que (in the past) + subjunctive

Hasta que (in the past) + subjunctive

¿Son correctas estas tres frases?:

No me levanté hasta que el examen empezara.
No me levanté hasta que el examen hubiera empezado.
No me levanté hasta que el examen había empezado.

En cuanto la última frase con 'había empezado', no estoy seguro.
