prestar declaración = to make a declaration, to testify, to give testimony
tomar declaración = to receive testimony?

I think both are ok.

La policía tomo declaración a 4 testigos.
The police took statements from four witnesses.
The police received testimony from four witnesses.

I think "to take a statement" would be what the police do at the scene, and "to receive testimony" would be in a courtroom.
