I'm not sure at all, but I think that it is "to fill out". Anyway, when I do my English exams, sometimes I'm asked to "fill in the gaps" (literal: llenar los huecos). So, I'm not sure if "to fill" is enough or you need "to fill out".

Either is possible in: fill in a form or fill out a form. But you can only fill in the gaps.

I never use rellenar for forms. I still think completar is better.
I would rellenar a pepper.

I would say "llenar una solicitud", "llenar un formato", "llenar una forma".
The word "formulario" is starting to gain some popularity because that's how Microsoft Office and similar software call what we used to know as "formas" and "formatos".

Speaking of rellenar........how do you say "refill"........as in refill a prescription?

I hear a lot of patients say "Quiero hablar a la farmacia para rellenar mi medicina" but it sounds off for some reason.
