What does this mean?

¿ …. o era la única cuerda?

Could it be figuratively limited to a single musical note.

It could be a way of saying or something else.

Because the adjective "cuerdo(a)" comes from the noun "cordura" (sanity), which is derived from Latin "cordis" (heart), not "chorda" (string).
