does this mean down at the heals (in other words :not doing too well financially)?
Is this an international phrase?

Correct. Although it does not relate only to the financial field.

"Por los suelos" is like "really down", and as Hernán said, in many fields, not only in the financial business.

Tengo el ánimo por los suelos.
I'm feeling really down.

Los precios de las acciones están por los suelos.
Shares prices are really down.

Mi dignidad quedó por los suelos cuando le ofrecí un soborno.
My dignity was so low down when I offered him a bribery.

La credibilidad de la empresa está por los suelos. Nadie cree que pueda pagar su deuda.
There's almost no credibility on the firm. Nobody thinks they can pay their debt.
