And it's the same as "(caer como) un balde de agua fría". In Mexico it's "una cubeta(da) de agua fría".

It describes an unpleasant sensation that paralyses/freezes you, usually caused by a surprise.

Expresiones sinónimas, en mayor o menor medida:
caer como una bomba,
causar buen [o mal] efecto,
dejar [o quedarse] frío,
dejar [o quedarse] helado,
causar [hacer o producir] impresión, impresionar[se],
dejar [o quedarse] como un muerto,
dejar [o quedarse] patidifuso,
dejar [o quedarse] de una pieza,
dejar [o quedarse] sin respiración,
sorprender[se],
dar un vuelco el corazón.
ser algo descacharrante,
ser algo epatante,
ser algo que tira de espaldas,
dejar [o quedarse] como una estatua...

Obviamente, no todas tienen el mismo "toque" pero son similares en tanto que causan sorpresa/impresión. La expresión original, tiene sus pros y sus contras, dependiendo de si es verano en el Sur de California, o si es invierno en Siberia...
