Señor Holmes, quiero que venga usted a Makleton conmigo en el primer tren mi amigo sacudió la
cabeza

In this passage the sentence appears to start with a direct quote from a character in the book in first person, but then concludes with what looks like Watson narrating to the reader. How is it possible to mix these different speakers up into one sentence?

Oh no, it is not possible. There's definitely some missing punctuation.

I would write something like this:
"Señor Holmes, quiero que venga usted a Makleton conmigo en el primer tren." Mi amigo sacudió la cabeza.

Or like this:
—Señor Holmes, quiero que venga usted a Makleton conmigo en el primer tren.
Mi amigo sacudió la cabeza.
