Bad English = Bad Translation?
Is this sentence written wrong or ?
I can't seem to get a good translation in my head. "the purpose of this release is to collect payment of whatever benefits may be due me from my insurance plan(s) as a result of services provided by the hospital". Who's getting the benefits? The patient? or the hospital? What the document is trying to say is that .... "the patient consents to his medical records being released to the insurance company for payment of his bill". Is this what you understand by the statement above or am I missing something? Or am I totally duh this morning? Thanks again for your help!

Yeah, then I'd be pretty sure that it's a request to give them the right to bill the insurance company / companies. But I could be totally wrong.
