What is he like?
is it a question to ask also about the appeareance of sb or just about his character?

The title is wrong. Either What is he like? or What's he like?

It could be either physical appearance or character, but anybody with bothers with being unambiguous would say What does he look like? for physical appearance.

Usually "What is he like?" or "What's he like?" is referring to the personality type aspects (smart, funny, mean, etc). It can be looks, but it would be be more common to ask "What does he look like?" as Perikles pointed out.
