"¿Qué tal?" is short for "¿Qué tal estás?? or "¿qué tal va todo?"

It's one of those questions people ask without really being interested in what's going on with the other person.

99% of the people answer "muy bien", "bien", "genial" even if they are not fine or things aren't going well..
