Is John the one who likes football?

¿Juan es él que le gusta el fútbol.
¿Juan es a él que le gusta el fútbol.

¿Se necesita la palabra ‘a’?

"¿Juan es al (a + el, sin acento) que le gusta el futbol?"

Sí, se necesita la preposición, porque la fórmula general es "gustarle algo a alguien".
