We aren't a free translation service. But, since you provided your own attempt, I've provided some hints below.

Se debe tener
(I added the impersonal subject pronoun 'One', just to make your translation more complete.)

un formulario válido de una identificación
(You used the wrong word for 'form', as in "a form of identification." The word 'form' isn't in the original, so don't try to translate it.
I reminded you to use an indefinite article before the word 'identificación'.
After 'identificación', include the Spanish word for 'valid', but make sure its gender agrees with the noun it modifies.)

del cuadro
(Here you've chosen the wrong word for picture. Use the shortened word for 'photograph' instead. This word will follow the preposition 'con' instead of 'del'.)

() los terminales
(Replace the parentheses with the translation of 'to play at', using the preposition 'para' for the word 'to'. Check the gender of the noun.)

de.
(NEVER end a Spanish sentence with a preposition.)
Give it another try and we'll let you know how you did.

yeah sorry... compromiso... or is it perdoname?

The original original was very long and proper lawyer speak. How about:

Debe tener una identificación válida para jugar en los terminales.

I do need to find a pay-per-phrase service though before spring time. I have a car dealer that wants me to produce mailers in spanish.

Good job, but you've left out 'with photograph' and didn't correct the last noun's article.
