Hola a todos! Quisiera preguntar con qué frecuencia se usa la frase hecha "ser peor que un grano en el culo" o "ser un grano en el culo". Gracias por vuestras sugerencias.

It's interesting that in Spanish very unpleasant people are sometimes called described as an affliction. In Colombia, you may call someone a gonorrea, if you hate them. I don't believe this type of comparison is ever used in English.
