What is meant by the bolded phrase? I get that they're saying Ana doesn't feel the same and am guessing this is an expression of sorts?

Oh, Rusty replied before I posted this, but one more answer won't harm, I guess.
You are right, this is a set phrase. In this context, it means to have doubts, or to be afraid of something.
