When sb is a pain in the neck he or she is a tocapelotas. Can we say he is a ballbreaker.?I think in some countries with strong Spanish influence, this word has been translated into English like that.
But there is no BrE equivalent, and "pain in the arse" is the usual strong form of "pain in the neck"

Ballbuster is American English, a little vulgar though.

The word poli offered is the AmE form of the OPs word, but neither means 'a pain in the neck'. The AmE form of Perikles' expression, and his expression, are more likely candidates.
