Shouldn't this be When I have a headache, I can hardly do anything ?? ??

If it's can't do hardly anything, then isn't that a double negative? :thinking:

what would be the word for head as in for example..
the head of the team
the head of the family.

like a leader.

There may be more equivalent expressions, but these are the ones I can think of:

- Cabeza/líder/jefe del equipo = head of team
- Cabeza de familia = head of family

I would use "dirigente" rather for higher rank posts... "dirigente de un país" ≈ "cabeza de Estado" = "head of State"

To me: "Tengo solo un poco de pelo en mi cabeza" sounds better. Is the same but sounds better. Again, to me.
