I agree with the situation when we would use "últimamente".
A Mexican would just ask "¿Cómo has estado?". The verbal tense already expresses "lately" for us.

Does it always convey the meaning of "lately"? Or are there other meanings it can convey?
Also, I am not familiar with "ido", but that appears to me as a progressive ending attached to nothing. Is it a word on it's own in this case?

"Estado" is the Past Participle of "estar". The verb tense is Perfect Preterit which is formed by the auxiliary haber + Present Preterit in Spanish.

Yes, it generally conveys the meaning of "lately", "recently".

"Ido" is the Past Participle of the verb "ir" : "Yo he ido" = "I have gone".
