What does this mean?

¿ …. o era la única cuerda?

Could it be figuratively limited to a single musical note.

…. or was it the only string? Literal translation.
Context is needed.
It could be a way of saying or something else.

The only sane one

Would fit in context

Something like
Are you crazy or am I the only sane one

I can’t see how to get there from
Was it the only string

It works that way in English too. Consider, the only nut:D

Because the adjective "cuerdo(a)" comes from the noun "cordura" (sanity), which is derived from Latin "cordis" (heart), not "chorda" (string). ;)

"She's the only person who isn't nuts around here", something like this.
