El día de hoy (to talk about today)

El día de hoy es jueves.
Today it's thursday.


Al día de hoy (to talk about something accumulated till today)

Las víctimas del temblor suman ya varios centenares de muertos al día de hoy.
The victims of the earthquake add to several hundreds of dead people today (and counting).
