I understand this expression as when you refer to some piece of art, (a poem, a speech, a painting... a video) that really affects you profoundly, and really touches your heart...
Is that a correct interpretation?
Or is it something like it hits you on your "forefront" and/or your "brain" and you get it, having a total realization, and/or understanding of the matter at hand?

Te llega al alma...?
Te hace ver la realidad...?
Esto te pega un zambombazo...?
Esto da en el blanco...?
It reminds me the pun on the baseball... (or soccer ball...)
