En principio, me parece que quiere decir "directo", "sin intermediación", sin nada que amortigüe el efecto de algo, pero ¿cuál es el contexto, Poli?

I think(though I'm not sure of this) paños caleientes means kid glove treatment. To be handled with kid gloves means gentle treatment.
Example:
Francisco Correa presunto cabecilla de la trama Gurtel se lo resume sin
paños calientes a su abogado.

"paños calientes" (literally "hot towels/rags" more or less) where aplied to the mother in the moment of giving birth, or to ill persons, in order to lesser the pain/suffer.
Saying/doing something "sin paños calientes" mean to make it direct, blunt, without lessening the impact/pain. (see Tomisimos & Angelica).
