(A)fuera de moda
 
Yesterday I heard this phrase. Does it mean something like "to be in/out of style"?

I've never heard "fuera de moda", so I'll let Ookami to give you an accurate answer on it.

We use "pasado de moda" to talk about clothing (or attitudes) that belongs to a former fashion.

En Chile se usan las dos formas. Pasado/fuera de moda = out of style

No, sólo "de moda" = In (style)

Yes in Spain we use the following,
A la [última] moda. Conforme al gusto del momento: ‘Un sombrero a la moda. Viste siempre a la última moda’. A la última. [with 'moda'/fashion left understood; ie. "the latest"]
De moda. Se dice de lo que está de moda; que se lleva o usa mucho en el momento: ‘El color de moda’. Actual, moderno. [current, modern]
De última moda. [the latest in fashion]
«el último grito de la moda». [the last cry in fashion]
Pasado de moda. Anticuado. [old-fashioned, antiquated]
Pasar[se] de moda. Dejar de estar de moda. [not being fashionable anymore].

No está de moda ser concienzudo... pero a mí me gusta profundizar... :)
