STEAK - medium well with mushrooms and onions sauteed in butter.

Bistek - (como se dice - Medium well?) con champiñones y cebollas guizadas (sauteed) en mantequilla.

Con un buen antipasto de coliflor, zanahoria, aceitunas etc (todos los vegetales en escabeche), queso y algun pedazo de salchicha o salchichon............

That sounds really good too :) Creo que sauteed is sofreíto, pero medium well, no tengo idea.
