Do you think that speakers from other parts would be mostly likely to guess (even out of context like: Mi camisa era completemente gratarola), or most likely not to understand?

Does it have to agree with the noun, or does it not change form?
Do *gratarolo, *gratarolos, *gratarolas, exist as well?

"Gratarola" es un adverbio y no creo que alguien pensara en usarlo con "ser" después de un sustantivo y lo modificara a su vez con otro adverbio como "completamente". Además no significa "gratis" sino "adquirido de un modo que no implica desembolso, excluyendo regalos, hurto y robo". Las promociones 2x1 y otros "con su compra le regalamos..." tampoco son "gratarola".

Las cosas "se consiguen gratarola" o "te salen gratarola", o "es todo gratarola", y muy pocos usos más. Habrá unos 5 millones de personas que lo usen y unos 30 o 40 millones que lo entiendan.

Nunca en mi vida había oído esa palabra.

Según el DRAE se usa en Uruguay y quiere decir "gratuitamente".

Not a clue. I didn't know what it was until it was explained here. Frankly, I don't think many people will understand that word.
