Which is the stressed syllable on 'Alvaro', 'Alvarez' and possibly others? I believe it to be on the first "A" although I've never seen the written accent applied. Please advise.

Both should be written with an accent mark.

@Sancho: Old typing machines placed the accent mark superimposed on the capital letters, so the norm was that they shouldn't be accented, for reasons of aesthetics. And this was extended as a rule in other kinds of writing. Yet, the RAE, as Rusty rightly pointed out, has always said the capital letters must be accented.
