significa que está envuelto en algo que no entiende.
por ejemplo : un proceso legal
un curso de latín
una relación con una person complicada

En español uso está metido en un baile de gallinas, pero estoy casi seguro que hay otro modo de decirlo.

Nunca había oído baile de gallinas, pero me gusta.

And Rusty your example of "metido en algo hasta el cuello" seems right to
me.
