No estar en su sano juicio
 
It means to be insane, to do crazy things for example to eat grass in a park.
How do we say in English " no estar en su sano juicio?
He is not sane????
He is not mentally healthy???

He isn't compos mentis.

Yup, I heard too, he is non compos mentis.

What about, he is out of his mind? (Is that more like a temporary state or condition?)

"out of his mind" could be a temporary state, like high on drugs. The non compos mentis is a legal expression for a more permanent state.
