I could only think of 'have it together upstairs.'
A few others, with 'apto' in mind, are 'working with a full deck' and 'having all one's marbles.'

He's got it together upstairs.
He has all his marbles.
He's working with a full deck.

Both can be used in the negative to indicate just the opposite.

He doesn't have all of his marbles. He's lost his marbles.
He hasn't got it (all) together upstairs.
He's not working with a full deck.


A site on the web gives 'have a good head on one's shoulders' as one meaning of the idiom in your title. It also lists 'have one's head screwed on (the right way).'
Both are close to 'have common sense.'
