Parte de baja is when you go to the doctor, you call in sick and he gives you a piece of paper you send to the place you work. Parte de alta, is quite the oppossite. |

Parte de alta: no conozco una frase inglesa correspondiente, así que tendría que describirlo: a note from the doctor saying you're well enough to go back to work. |

Just for the record, in Mexico none of those expressions would be understood:
Parte de baja => incapacidad (el médico dice que alguien está enfermo y le firma su incapacidad) Parte de alta => alta (el médico da de alta a alguien y le da su alta) |
