You're asking a question that probably has as many answers as there are people.
The best way to become fluent in Spanish, or any other language for that matter, is to completely immerse yourself in it. This is best done by moving to one of the countries where Spanish is the primary language spoken by the people.
If you can't move away for a couple of years, the next best thing is to befriend native speakers of the language and always try to engage in Spanish-language conversation.
One of our members suggests that you translate a novel written in Spanish into English.
This helps you learn how the language is used. I would add that you should say the sentences aloud before you translate them into English. That will improve your ability to articulate the sounds.
You can listen to Spanish media. This helps with listening skills.
You can practice what you're learning here. This helps with writing and grammar skills.
In a nutshell, speak with someone as much as possible in Spanish. You won't achieve fluency otherwise.
