Yes, as you have said, there're diversity of ways in that you can give an expression for a only word.
In the old case that you said. What is going on? What's happening? Really they mean the same solely that in different forms. That was your question?

What was going on?
¿Qué pasó? [what happened?] (Although some people may use this as "What's up?") ¿Qué estaba pasando? [what was happening?] ¿Qué te traías? [what were you up to?] ¿Qué onda? [what's up?] (I think you only use this in present tense... there is not even a verb in Spanish here) Hope that gives you an idea. (Not very common to use these past forms, although you could heard them.) ![]()

Hey, and if you want to be "challenging" you could also say,
¿Qué demonios está pasando? (As in "what the hell is going on?") (Literally, "What the demons is happening?") ![]()
