Technically, Spanish doesn't have hyphenated words, yet this is what I saw. Since Spanish doesn't usually have hyphenated words, it's hard to cite any rules.
I've seen titles that have only the first word capitalized, but there are an increasing number of folks who are capitalizing each word like we do in English. In English titles, only the initial letter of a hyphenated word is capitalized.

"Escala de Evaluación Pre-lenguaje", sin el segundo "de". Con el "de", lo evaluado es el "pre-lenguaje", sin el "de" lo evaluado es la persona supuestamente antes de aprender el lenguaje. I think that the best translation for this is: " Escala de valoración prelocutiva"
Prelocutiva means: before begining to speak (to develop the lenguage in children)
