Right, nice work. However, may be easier if you sort the lines into six. For that, in modern spanish, we use the @ to mean male/female. So, how about:
-AR infinitivos such as bailar, volar, amar yo -o tú -as él/ella/Ud. -a nosotr@s -amos vosotr@s -ais (this one, the pl 'you people', was not on your original list) ell@s /uds -an |

It's certainly used. I found hundreds of thousands such uses on the Internet.
I would say it's safe to use it as a learning or teaching tool. We all know that only one or the other vowel should be used when speaking and officially writing in Spanish. |
