Despacio and lentamente
both mean slowly. how do you know which one to use?

one is an adjective and the other is an adverb

Both are adverbs, and I'd say they're synonyms. "Despacio" is used more often.

El auto pasó lentamente. / El auto pasó despacio.
The car passed slowly.

Háblame despacio para que pueda entenderte. / Háblame lentamente.
Talk to me slowly.

Conté las monedas lentamente. / Conté despacio las monedas.
I counted the coins slowly.

Slow = lento/despacio

Slowly = lentamente

Unless it were a regional use... but as far as I know, no, it can't... There should be "despacia", "despacios", "despacias".

Juan es muy despacio lento. Nunca hace nada rápido.
Juan is very slow. He never does anything fast.

Soy tan despacia lenta para arreglarme, que nadie quiere esperarme.
I'm so slow for dressing up, that nobody wants to wait for me.

Me aburro en las despacias lentas horas de la tarde.

No es que estén mal, pero se usa más en la manera que describes.

Fue la primera que traduje, la segunda "¿me lo puede repetir por favor?", la tercera "todavía no entiendo, ¿me lo puede decir de otra manera?"

DRAE gives despacioso as the cognate adjective.
