Según una búsqueda google significa cream of the crop. Quisiera saber si

It is definitely widely used in Spain. REAL ACADEMIA ESPAÑOLA: Banco de datos (CREA) [en línea]. Corpus de referencia del español actual. http://corpus.rae.es/creanet.html They give many Spain references, one usage from Mexico, and none from the rest of Spanish-speaking areas. So it seems to be an usage circumscribed to mainly Spain.
