Mal que no mejora empeora

@Robin: Por favor toma en cuenta que varias de las expresiones por las que preguntas no son conocidas por todos los usuarios, así que se necesita que expliques más y dés ejemplos para poder ayudar.

En este caso, más que "modismo" es un refrán que como María Moliner dice a veces en su diccionario es "de significado claro".

That is,
A bad thing [illness, wrong, suffering, an evil, problem, misfortune...] that does not get better (does not improve), it gets worse.

The idea is that "nothing remains in a stable condition in this universe".

Unless you are improving, you are going to get worse.

My French is pretty poor nowadays as I have not practiced for yeeears... so unless I start to practice, it will get worse.

Or "Peter has a chronic sinusitis and since he is not treating it and taking proper care, it's getting worse..."

La cosa va de mal en peor = This matter is going from bad to worse.

Salir de Guatemala y meterse en Guatepeor...

(Uy, que no me den cuerda, que si no paro, la cosa va a degenerar...)
