In Spain, espinilla is blackhead?

In Chile espinilla is a zit and blackhead is "barro" or "punto negro" :-)

zit, pimple = barro, barrito
blackhead = punto negro

Pimple was the other word i forgot to write. However, I did not know that barrito was the name for pimple in Mexico...

Also the word Espinilla is named as grano in Mexico, but I don't if am in the correct the word grano would be translate as grain?

Yes, grain is one of many meanings of grano.

grano (defecto en la cara) - zit, pimple
grano (trigo, avena etc) - grain
